Home Care Services

The services we provide under the Home Care
Package, NDIS plans and private funding. We are very flexible when it comes to you choosing who comes into your home to support you.

Domestic Assistance

We’ll make sure that your house is kept how you like it. Our team is there for you to lend a hand with odd jobs around the house.

Garden Maintenance

No need to worry about Pruning, Mowing and weeding, we are here to help with all of the heavy lifting.

Meal Preparation

Our team can work with you to prepare nutritious and tasty meals, following your favourite recipes. We can also arrange meal delivery assistance if you’d prefer.

Personal Care

Our staff understand the importance of your independence while providing the support you need in taking a shower and getting dressed.

Out & About Services

- Transport: If you need assistance in getting to an appointment or social event, we are here to help.
- Social Support: Being Able to do activities you love is an important thing.
Our team is here for you when you’d like companionship over a cup of tea or someone to accompany you to a social function. Or to day-to-day activities such as shopping, library, hairdresser, attending the gym and swimming.

Dementia home care

We provide high quality at-home Dementia Care and Disability Services; this service is for people living with dementia or disability at their home to provide a 24-hour care support where it’s often needed the most.
After an Initial Consultation, Home Caring offer you a comprehensive Overnight Respite & Palliative Care Plan which can be tailored to suit all of your needs and requirements for providing the aged care & respite care support you need in your home.

Creativity

Through art and woodwork workshops we encourage creativity. Throughout the years you can take part in art exhibitions at so many venues. Art and craft products like cards, design, soap and more

Social and Living Skills

We can help you take part in activities such as shopping, swimming, hors riding and more as an individual or in a group.

Drop in Support

Ability Support and Care supported living provides regular drop-in support to people who need a little bit of support to live independently in the community, like adults with an intellectual disability who live on their own homes.
Visiting workers provide you with support and training in numerous areas depending on your need.
They can help you with
• Social Skills - Support in: Self-esteem, Self-advocacy, Communication, Relationships, Making decisions and Choices.
• Money Management – Help in budgeting, banking and paying the bills
• Leisure Activities – Support in Accessing holiday activities.
• Personal life – Assistance in making and keeping appointments and
maintaining healthy lifestyle.

Home Modifications

Home modifications help people to live safely and as independently as possible, they include design and construction also equipment and fixtures fitting or changes to non-structural parts of the building.

We can assist you with quotes and eligibility check.

Specialist disability accommodation

Specialist disability accommodation (SDA) is for people who need specialist housing to support their high needs. SDA may include the special designs for people with very high needs, or in a location more suitable to provide thorough support.
Areas of SDA we can assist you with are:
• Access to registered SDA units and houses.
• SDA person-specific adjustments.
• SDA support collaboration.

Transport

Transport is very important to access the community on day to day bases, it is funded under the National Disability Insurance Scheme (NDIS) as an annual amount and paid in advance to clients; fortnightly.
We offer transport in our cars and busses so you can improve your skills and reach your NDIS goals.
Our transport includes:
• To and from work.
• To and from appointments, courses and shopping.

Community Nursing

We can provide support to participants who have complex support needs though our community nurse who has medical experience in the disability space.
This service is used when:
- Complex Bowel Care
- Feeding and Management
- Medical needs or procedures like Naso-Gastric Tube-Jejunum or Duodenum
- Tracheostomy Management
- Subcutaneous Injections
- Complex Wound Care and Management
- Urinary Catheter Management
- Day-to-Day Management of Medications

Specialist Building

At Ability Support and Care we are dedicated to support you increasing your capacity to be as independent as possible through developing living skills to help you through your day today life such as:
• Personal Care including:
o Showering and Dressing
o Menu planning, cooking and shopping
o Budget planning
o Booking and attending medical appointments
o Management of written communication
o Medication management
o Co-ordination of other support workers like cleaning
Support can be provided at one’s own home or in a shared environment

Construction Design

We understand how important it is for those living with a disability to stay safe and independent in their homes, we also appreciate the need to modify the home environment to enhance one’s capacity and needs.
We are proud to be able to improve the lifestyle, safety and mobility of our participants. We provide you the best solutions to meet your needs of everyday living through working with a strong network of approved suppliers.

Modifications can be simple or complicated depending one’s needs, they may involve building a ramp, widening doors, modifying kitchens and bathrooms, installing grab rails, putting in environmental control units or automating one’s home.

We will help you understand the NDIS process and answer any questions you may have.

It is a requirement under the NDIS that an Occupational Therapist is involved in the providing of any Assistive technology equipment, minor home modification and major home modifications.
Your Occupational Therapist will:

  • Come and meet you and your family to learn more about your needs
  • Complete the initial home assessment
  • Complete the NDIS report (using the required acceptable template)
  • Create a scope of works for the modifications and/or equipment script
  • Source quotes from builders, contractors and equipment suppliers
  • Provide a draft of the application for your approval prior to submitting the application to the NDIS
  • Submit the application to the NDIS
  • Answer any questions the NDIS have regarding the report and recommendations
  • Liaise with the builder throughout the building process
